Anasazi is a nonprofit behavioral healthcare provider licensed in the State of Arizona and accredited by The Joint Commission. This Privacy Statement explains how we collect, use, disclose, and safeguard information obtained through our website and how this relates to our obligations under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and applicable state laws.

BestNotes acts as a business associate of Anasazi and is contractually required to safeguard Protected Health Information (PHI), including maintaining appropriate administrative, technical, and physical security measures.

Use of BestNotes is governed by Anasazi’s HIPAA Notice of Privacy Practices, not solely by this website Privacy Statement. Individuals seeking services may receive additional information about BestNotes during the admissions or intake processes.
