Licensing & Accreditation
Anasazi Foundation is a 501(c)3 non-profit organization serving youth 12-17 years old with substance abuse and emotional and behavioral concerns. Anasazi is licensed by the Arizona Department of Health Services, Office of Behavioral Health, as a Level II behavioral healthcare provider (residential) and is accredited nationally by the Joint Commission on Accreditation of Healthcare Organizations (Joint Commission). Anasazi’s clinical staff members also maintain individual licensure.
The professional staff organization at Anasazi includes a psychiatrist, clinical social workers, professional counselors, certified addiction counselors, and registered nurses. Treatment at Anasazi is under the supervision of a clinical director utilizing a case-manager system. The minimum direct staff to patient ratio is one to three. Group sizes range from three to nine.
Anasazi maintains a quality assurance/utilization review program to monitor quality and effectiveness of treatment. Weekly staffing meetings are conducted to evaluate progress and review treatment plans for each patient.
Anasazi has relationships with urgent care facilities and Joint Commission-accredited hospitals for diagnostic (including x-ray and laboratory) and emergency (including major surgical) services.
