Licensing & Accreditation

ANASAZI Foundation is a 501(c)3 non-profit organization serving youth 12-17 with substance abuse and emotional and behavioral concerns. ANASAZI is licensed by the Arizona Department of Health Services, Office of Behavioral Health, as a Level II behavioral healthcare provider (residential) and is accredited nationally by the Joint Commission on Accreditation of Healthcare Organizations (Joint Commission). In addition, ANASAZI maintains Outdoor Behavioral Healthcare Accreditation offered through the Association for Experiential Education (AEE). ANASAZI’s clinical staff members also maintain individual licensure.

The professional staff organization at ANASAZI includes a psychiatrist, licensed clinical social workers, licensed professional counselors, certified addiction counselors, and registered nurses. Treatment at ANASAZI is supervised by the clinical director utilizing a case-manager system. The minimum direct staff to patient ratio is one to three. Group sizes range from three to nine.

ANASAZI maintains a quality assurance/utilization review program to monitor the quality and effectiveness of treatment. Weekly staffings are conducted to evaluate progress and review treatment plans for each patient.

ANASAZI has relationships with urgent care facilities and Joint Commission-accredited hospitals for diagnostic (including x-ray and laboratory) and emergency (including major surgical) services.
